How can my agency join?
- Contact Judy Milone, Assistant Director (212) 801-1350 to receive a membership package or download an Agency Membership Application pdf (120506 bytes).
- Submit the completed application with the following items:
- Certificate of Incorporation
By-laws
- Description of programs
- Current budget
- Most recent audit
- List of Board of Directors
- State Tax Exempt form
- Federal Tax Exempt form
- Once your completed application is received, an FPWA staff member will arrange to visit your site and meet with the executive director and Board chair.
- The agency application and site report is reviewed by FPWA’s Program Member Services Committee. The Committee then makes a recommendation to the full FPWA Board.
- A screening of agency Board members and senior staff is conducted through the Department of the Treasury Office of Foreign Asset Control.
- Upon acceptance by the FPWA Board, the agency is welcomed to FPWA and a membership certificate is issued. At this time, annual dues are paid.
Membership fees - our annual membership fees are based on the size of an agency's budget, as follows:
- Under $1 million: $250
- $1 million to $2.5 million: $400
- $2.5 million to $5 million: $600
- Over $5 million: $900
